Admission Checklist

ADMISSIONS CHECKLIST

Checking into a hospital may be a scary experience for some, but at West Shore we will put you at ease. We know that you are away from home, but we will try to make you feel at home with us. You will be required to walk with the following.

  • Admission Letter from your doctor

  • Medical Documents – ALL medical documentation pertaining to your medical problem (x-ray results, CT scan results, blood test results, ultrasound results, etc.)

  • Medical Insurance information and Medical claim forms if any

  • Financial Information – Financial sources to cover admissions. See our Payment Policy

  • Personal Documents – A copy of personal identification (ID Card, Driver’s Permit or Passport)

  • Clothing – pajamas, underwear, discharge clothes, slippers, socks, reading glasses etc. Please note that these should be loose with easy access to incision site (if any)

  • Toiletries – toothbrush, toothpaste, soap, shampoo, powder, towels etc.

  • Personal Items – e.g. a book to read.

CHILDREN

When admitting children, we recommend that parents provide them with an item that is special to them so that they can feel comfortable. This may be a favourite toy such as a doll, a teddy bear or even a favourite book. Noisy toys are discouraged.

DISCLAIMER

Kindly leave your valuables at home. If you happen to be in possession of any valuables, they should be given to close family members and loved ones for safekeeping. Failing which, please advise the Floor Manager on duty of any items you may wish to lodge with the Nursing Station, where a log will be kept of your items. West Shore Medical Private Hospital will not be held responsible for the loss, damage or theft of items left in your room. Valuables include: jewellery, large amounts of cash, family photo albums, etc.
