Checking into a hospital may be a scary experience for some, but at West Shore we will put you at ease. We know that you are away from home, but we will try to make you feel at home with us. You will be required to walk with the following.
Admission Letter from your doctor
Medical Documents – ALL medical documentation pertaining to your medical problem (x-ray results, CT scan results, blood test results, ultrasound results, etc.)
Medical Insurance information and Medical claim forms if any
Financial Information – Financial sources to cover admissions. See our Payment Policy
Personal Documents – A copy of personal identification (ID Card, Driver’s Permit or Passport)
Clothing – pajamas, underwear, discharge clothes, slippers, socks, reading glasses etc. Please note that these should be loose with easy access to incision site (if any)
Toiletries – toothbrush, toothpaste, soap, shampoo, powder, towels etc.
Personal Items – e.g. a book to read.
